The surgeon oversizes the wound when manufacturing the main entry and when attempting to use the phaco tip too much fluid escapes the eye making for an unstable AC: Solution:

1.Raise the bottle height for more infusion/inflow.

2.Surgeon to place a 10-0 nylon suture at the end of the wound to shorten its chord length.

The patient is using Flomax©, an oral drug notorious for making the AC unstable and the iris floppy during Surgery: Solution:

1.Your surgeon may opt to use “iris hooks.” Your job would be to supply your surgeon with a toothless Kelman-McPherson type forceps in order to remove each hook from the foam board they are imbedded in. Alternatively, you would perform this function.

The surgeon will first use a 15° angulated blade to make a series of entries in the peripheral cornea in order to gain entry into the AC.

Your surgeon will manipulate each hook through the wound to catch the edge of the pupil, then draw the pupil edge towards the corneal wound and then use the small block of plastic at the end of each hook to “lock” the system in place against the cornea.
